The oldest thread goes back to 1921, when SSV Mülheim first laced up. A 1992 merger with FC Kärlich brought the two local clubs under one roof, and by 2000 that side had joined forces again, this time with SSV Urmitz-Bahnhof, playing on as SG 2000 Mülheim-Kärlich until the arrangement was formalised into a single club in 2011.

A first Amateurliga Rheinland title in 1967 sent the club up into the Regionalliga Südwest, then the second tier of German football, though the stay lasted only a season before relegation dropped them back down. A second Rheinland championship followed in 1969.
