Górnik Polkowice reached the Ekstraklasa for the 2003–04 season, and lasted a single campaign there: fourteenth place, then relegation after a play-out defeat to Cracovia. That same season the club got to the quarter-finals of the Puchar Polski, with Marcin Narwojsz top scorer, a high-water mark the club has not touched since. Three years earlier they had gone as far as the semi-finals of the Puchar Ligi.

The name on the badge has changed more often than the club's fortunes. It started out in January 1947 as KS Włókniarz Polkowice, and passed through Ludowy Zespół Sportowy Polkowice and a run of "Górnik" variants before a spell as a limited company, KS Polkowice sp. z o.o., from 2011. In 2018 the club took back its old identity as KS Górnik Polkowice.

2007 cost the club two divisions at once, when a corruption scandal saw it relegated straight down.
