In 1993, Ethiopian Medhin reached the semi-finals of the CAF Cup, a run that ended only when Stella Club d'Adjame shut the door on them.

The club still carries the name Ethiopian Insurance Football Club, and its nickname, Medhin, means "insurance" in Amharic. Their first recorded appearance in the national championship came in 1984, when they finished runners-up.

Domestic silverware has come in short bursts rather than long stretches. The Ethiopia Cup was won in 1995 and again in 2002, the second final settled on penalties against Mebrat Hail.

They returned to the continent twice more without matching 1993: a first-round exit against al-Ahli Khartoum in the 1999 CAF Cup, and a heavy 0-4 defeat to al-Hilal Khartoum in the 2003 African Cup Winners' Cup. Medhin play their home games at Addis Ababa Stadium, in the city's Kirkos district.

The 2025 season brought their first Ethiopian Premier League title, a result that put them back at the top of the domestic game.
