The Gradski stadion Velimir Sombolac holds 5,000, and Kozara Gradiška have played out most of their history there in red and white. The club took shape in June 1945, and it has stayed tied to Gradiška ever since.

Its best season on record came in 1973–74, when the side finished third in the Yugoslav Second League. Domestic silverware followed later: the Bosnia and Herzegovina Republic League in 1980–81, and, after Republika Srpska set up its own football pyramid, the First League of the Republika Srpska title in 2010–11. Kozara Gradiška also went through a strong cup run at the turn of the millennium, lifting the Republika Srpska Cup in 1993–94, 1999–2000 and 2000–01.

The club now plays in the 1st League of the Republika Srpska, competing for a return to the top flight against clubs from across the region.
