The goal came in 2025, the season Always Ready won the Bolivian title, and it remains the only one Damián Medina has scored for the first team.

He came up through the club's own academy and never left. Six matches for the U20 side from July 2023, no goals, and by then he had already been on the pitch with the seniors: his debut in the Bolivian Primera came on 5 March 2023, against Jorge Wilstermann. Because he is a product of the academy, no fee was ever involved in the move up.

Progress was slow at first. Two appearances in 2023, three in 2024. The change came in 2025, when he played 13 matches and scored. Always Ready took the División Profesional that year on the strength of the regular season: 71 points from 30 matches, 22 wins, five draws, three defeats, the best return in the club's history.

That same year he was called into Bolivia's under-20 squad for the Sudamericano Sub-20 and played three matches there. There has been no call-up to the senior national team.

The 2026 season has brought 12 appearances so far, and one that ended early: on 27 August 2026, against Aurora, he was sent off. Across his first-team career the count stands at 42 matches and the single goal.

He has stayed at one club for the whole of a career that is still short.
