«Bijelo-plavi»
The club now known as NK Osijek traces back to NK Udarnik, founded on 27 February 1945 from the banned JŠK Slavija Osijek, itself dating to 1916. A year later Udarnik merged with Jedinstvo and took the name Slavonija. On 27 February 1947, Slavonija combined with NK Bratstvo to form FK Proleter. The Slavonija name returned in 1962 during a reorganisation of the city's multi-sport association, and in April 1967, once that structure was dissolved, the club broke away as an independent football side under its current name.
Osijek play in white shirts and blue shorts, a combination that earned them the nickname Bijelo-plavi. Their honours list is short but not empty: the Croatian Cup in the 1998–99 season, and a runner-up finish in the Prva HNL in 2020–21.
Since 2023 the club has called Opus Arena home, a venue that replaced the old Gradski vrt as its ground.
