Fixtures against Alkaloid carry a name of their own in Macedonian handball, the "fraternal showdown," while the men's game's real Eternal Derby belongs to Vardar and Eurofarm Pelister.

As Ševro Kumanovo it took the first Macedonian Handball Cup in 1993.

The modern history has not matched that early ceiling. A third-place finish in the Super League playoffs in 2011–12 brought a return to European handball, this time in the EHF Cup, but two years later the club dropped into the First League for missing the deadline to convert into a joint-stock company. Eight seasons followed in the second tier. A fifth-place finish in 2021–22 was enough to bring Kumanovo back up, and it played the 2022–23 season in the Super League again.

Sports Hall Kumanovo, built in 1980, has staged all of this and remains the largest indoor venue in the city.
