The current club traces its name to 1991, when people from Vukovar founded a team in Zagreb after fleeing the fighting in their city. A year later it merged with RNK Sloga, the local side that had existed since 1946, and by agreement took on Sloga's founding date as its own. That version of the club folded in 2012 under financial strain; a new HNK Vukovar 1991 was formed the same summer and has carried the name since.

Promotion battles have defined the rebuilt club more than any single trophy. It took the 3. HNL East title in 2021–22 to reach the Prva NL, then won that division outright in 2024–25, going up to the top-flight HNL for the first time in its history. Vanja Pelko led the scoring column in that promotion season, while Josip Barišić , top scorer across the two previous campaigns, has since moved into coaching with the club. Ivan Krstanović, a forward with a long career in Croatian football, joined in the 2023–24 season and added experience to a squad still finding its footing at a higher level.

Home matches are played in front of just over 3,500 people at Stadion u Borovom Naselju. The Croatian Cup has never been won, by either version of the club.
