Sarajevo grew around the railway that connected it to the rest of Yugoslavia, and it was railway workers who founded the club on 19 September 1921, giving it a name that simply means "the railwayman." From the start it took players regardless of nationality, an open policy that drew resistance in the interwar years but stuck.

Football stopped in 1941 and resumed in 1945, and the postwar decades produced the club's only top-flight Yugoslav title, in 1971-72, under coach Milan Ribar. That same season carried the team into the UEFA Cup quarter-finals. Željezničar went further under Ivica Osim, a former player who returned as coach and took the club to the UEFA Cup semi-finals in 1984-85, still the best European run by any Bosnian side; Edin Bahtić finished that campaign as the competition's top scorer with seven goals. War reached Grbavica itself during the siege of Sarajevo, wrecking the stadium and halting the league until the club returned to competition in 1994-95.

Independence brought a run of dominance. The 1997-98 title, won under Enver Hadžiabdić, arrived through a match against city rivals Sarajevo still remembered as "Zubandan." Amar Osim, Ivica's son, then built the club's most decorated spell, winning titles in 2000-01 and 2001-02 and completing the first league-and-cup double in Bosnian football; a second double followed in 2011-12. Edin Džeko came through the academy before his career took him to Manchester City and Roma.

Grbavica now holds 13,057, and the club plays in blue and white, the colours behind its nickname, the Blues. None of it carries the weight of the fixture against Sarajevo, founded in 1946 when players and supporters split away from Željezničar. Their first official meeting came in 1954, and the derby has been called the Eternal Derby ever since — still the biggest occasion in Bosnian football.
