The rivalry Al Qadisiya carry in the women's game is not the one that draws the crowds in Al Khobar. Their men's side has history with Al-Ettifaq; the women's derby belongs to Eastern Flames FC, and it is that fixture, not the older city rivalry, that decides bragging rights in the Eastern Province.

The women's team did not start out as Al Qadisiya at all. It was formed in 2019 as Al Mutahed Women's Football Club, and only in August 2023 did the men's club, Al Qadsiah FC, rebrand it under its own name and colours. Promotion followed almost immediately: the side finished runners-up in the Saudi Women's First Division League in the 2022–23 season, enough to go up to the Premier League.

Since then the club has built a squad with a distinctly international accent. Icelandic midfielder Sara Björk Gunnarsdóttir and Cameroonian forward Ajara Nchout Njoya both signed in 2024, and it was Nchout Njoya who finished the 2024–25 campaign as the team's top scorer. Results in regional cup competition have improved year on year: third place in the SAFF Women's Cup in 2023–24, then runners-up in 2024–25. Luís Andrade took the side through that stretch as head coach before handing over to Carmelina Moscato in June 2025.

Home games are played in front of 11,000 at Prince Saud bin Jalawi Stadium, but it is the trip to Dammam, and the meeting with Eastern Flames, that the Eastern Province follows most closely.
