Three former Georgia internationals, Irakli Vashakidze, Soso Grishikashvili and Aleksandre Amisulashvili, put the club together after Kakheti, the previous professional side in the city, ran into financial trouble and dropped into the third division in 2014. Telavi were registered on 19 July 2016, and the climb that followed had little pause in it: the regional league that same year, a runner-up finish in the third tier in 2017 with a play-off win over Guria to move up, fifth place in the second division in 2018, then third place and a play-off victory over Rustavi in 2019 that put the club into Erovnuli Liga, the top flight, for the first time.

They stayed there from 2020 through 2024, opening with sixth place in that first season. In March 2023 the club took on its first outside investment, from Armenian businessmen Vartan Hakobyan and Tigran Hakobyan. The best cup run came earlier, a run to the Georgian Cup semi-final in 2018, ended there by Torpedo Kutaisi. Season 2025 brought eighth place and a play-off defeat, and relegation back to Erovnuli Liga 2.

Home games belong, in principle, to the stadium named after Givi Chokheli, which holds 12,000 in blue and white, the club's colours. Reconstruction work has pushed recent seasons onto the Kavkasioni Arena instead, a rugby ground with room for a few thousand rather than the thousands the main stadium can take. Few players who have worn the shirt came up through Telavi itself; most, like Giorgi Iluridze or Giorgi Kantaria, arrived already shaped by academies in Tbilisi, at Dinamo or Saburtalo, and passed through on loan or on their way somewhere else. A first Georgian Cup remains the one thing a decade of steady progress has not delivered.
