Two names run through Shimshon Tel Aviv's history. Beitar Tel Aviv was the other, and matches between the two were known as the Suburbs Derby, or Derby HaPeri'ot.

The club carried the name of Shimshon Rouzi, a Maccabi Tel Aviv player killed in the 1948 Arab-Israeli War, when it was founded a year later. It grew out of the Kerem HaTeimanim neighbourhood and was known simply as the club of Kerem HaTeimanim, playing its top-flight matches at Bloomfield Stadium and Ramat Gan Stadium. That original club did not survive the 2000 merger with Beitar intact, and the merged entity was dissolved in 2011. A group of local businessmen brought Shimshon Tel Aviv back in 2014, returning it to competitive football.

Gidi Damti spent his entire career at the club, 537 appearances and 69 caps for Israel. Moshe Romano was top scorer in 1965–66 and again in 1969–70, and good enough by 1970 to play for Israel at the World Cup. Vicky Peretz started here too, before he went on to become a Maccabi Tel Aviv legend.

Shimshon won the Second Division in 1959–60 and took the Toto Cup Leumit in back-to-back seasons, 1986–87 and 1987–88, but the State Cup kept slipping away: runners-up in 1966 to Hapoel Haifa, in 1986 to Beitar Jerusalem, and in 1990 to Hapoel Kfar Saba.
