The league sits at the top of the Italian basketball pyramid as the premier professional men's competition. It differs from the domestic cup by serving as the primary championship determined through a long season and playoffs rather than a knockout tournament. The structure includes a regular season running from October to May, where teams face each other in a double round-robin format for 30 games. The top eight sides then advance to a playoff stage featuring best-of-five quarterfinals and best-of-seven semifinals and finals. The winner of the final series receives the Scudetto.
Access to the top flight relies on promotion and relegation with Serie A2. The club finishing in 16th place moves down automatically, while the 15th-placed side enters a playoff against the Serie A2 winner to secure the final spot. Two teams arrive from the second tier: the league champion and the playoff victor. This system ensures a continuous flow of talent between the divisions.
Olimpia Milano holds the record for the most titles, having won the competition 32 times as of the 2025–26 season. The club also claimed 14 titles between 1949 and 1967 under coach Cesare Rubini. Virtus Bologna stands as the second most successful team with 17 championships. The league underwent significant changes in its history, including a split into Serie A1 and Serie A2 in 1974 and professionalization in 1994. It was renamed from Serie A1 to Lega Basket Serie A in 2013 following a merger. A notable incident involved Mens Sana Siena having titles revoked due to fiscal fraud despite winning seven consecutive championships from 2007 to 2013. The competition currently features 16 clubs and is regulated by the FIP.
