The reserve side was pieced together in 2006, not by growing a youth group into a squad but by buying the league licence of FC Dragoun Břevnov after that club folded at the end of the 2005–06 season. An earlier B-team had already existed once, until 1991, so this was a restart rather than a first attempt. The parent club goes back further, to a 1902 side called Studentská XI., though FK Mladá Boleslav only took its present corporate form in 1950.

The B-team plays in Group B of the Czech Third Division, and it has no senior honours of its own — the two Czech Cup wins, from 2010–11 and 2015–16, sit with the first team. Vojtěch Stránský spent four seasons there, from 2020 to 2024, before moving on to Slovan Liberec, and Matěj Pulkrab passed through on his way to the senior squad and later FK Teplice.

On matchdays the opponents are drawn from the same tier of Central Bohemian football — reserve sides from Sparta Kolín, Hradec Králové B, Pardubice B.
