The Phantoms had gone twenty seasons without a championship before they beat the Fargo Force 1-0 in 2023 to take the Clark Cup, the USHL's playoff title. The Anderson Cup, awarded for the regular-season standings, came to Youngstown in 2015 and again in 2026.

Bruce Zoldan put the franchise together in 2003 to bring Tier I junior hockey to the Mahoning Valley. It started out in the North American Hockey League, playing at a rink in Boardman under the name Youngstown Phantom Rockets, then spent four seasons as the Mahoning Valley Phantoms after a professional club took the Youngstown name for itself. In 2009 the franchise moved up to the United States Hockey League, the country's top junior circuit, and took back the name it carries now. Under the Mahoning Valley name it had already won the NAHL's regular-season title and its North Division in 2007, then lost the Robertson Cup final to the St. Louis Bandits.

Ownership has stayed close to the founder: Zoldan sold half the club to former NHL defenceman Troy Loney in 2014, and Black Bear Sports Group bought out the Loney family's stake four years later, with Zoldan still holding the other half.

Kyle Connor and Brandon Saad both came through Youngstown before the NHL; Saad went on to win the Stanley Cup with Chicago in 2013. Seventeen players who wore the purple, orange and black have been drafted into the NHL.

The Phantoms play at the Covelli Centre, a 5,200-seat arena in Youngstown.
