Foot Ball Club Melgar formed on 25 March 1915. It was initially called Juventud Melgar before settling on Foot Ball Club Melgar within three months, with Manuel M. Maldonado as its first president.

The club plays at the Estadio Monumental Virgen de Chapi, also known locally as the Estadio de la UNSA.

National titles have come slowly. Melgar won the Copa Perú in 1971, then waited a decade for the Campeonato Descentralizado in 1981, and did not add a second until 2015. The closest Melgar has come back to the top since was 2022, when they reached the Torneo Apertura final and lost it to Alianza Lima.
