The date most often attached to KK Mladost Mrkonjić Grad is 1970, though at least one outlet has pushed the club's origins back further. The earlier claim does not hold up: that distinction in Bosnia and Herzegovina belongs to KK Borac Banja Luka, founded in 1947, and nothing ties Mladost to that period beyond the assertion itself. The 1970 date carries the weight of consistency across the record.

Mladost plays in red and blue, a pairing fixed enough to have its own shade codes attached to the home and away kits, and its home is the Arena Komercijalne Banke in Mrkonjić Grad. Reported seating there has varied by source — one figure as high as 2,000 — but the number that keeps turning up, matched against actual attendance recorded during the 2012–13 season, sits close to 1,000, with 730 of those as fixed seats.

The club's best-remembered run came through the domestic cup competitions rather than the league table. In 2014, Mladost reached the final of the Bosnia and Herzegovina Cup and lost to Igokea, a tie still recalled as one of the club's defining nights. The following year brought the trophy it did lift: the Cup of Republika Srpska, won in 2015. Promotion to the Premijer Liga BiH came by way of the First League of Republika Srpska, taken more than once, and since arriving at the top level Mladost has made a habit of reaching the playoffs, semi-final appearances included, across more than ten seasons.

In 2017, the club was named among the entrants for the qualifying rounds of the ABA League Second Division's first season — a step toward regional competition rather than a step already completed.
