Before Peñarol had a basketball section, there was a side called Club Piratas, formed in the late 1920s. It became the yellow-and-black club's basketball team in 1931. The players wore the same colours as the parent multi-sport club and took its nickname, Carbonero, though some supporters also call the team Manya or Mirasoles.

The early decades held one odd interruption. In 1945 the club left the Uruguayan Basketball Federation for a parallel professional league. That league collapsed, and Peñarol rejoined the federation in 1947. By then the club had already won the federal championship in 1944, and it took the title again in 1952, then four more times between 1973 and 1982.

The peak came in 1983, when the team under coach Víctor Hugo Berardi won the South American Championship of Champions Clubs and qualified for the FIBA Intercontinental Cup. Jeff Granger, an American import who became a naturalised Uruguayan, played in Uruguay in those early-1980s years. His son Jayson later wore the same shirt after a career in Europe.

The decline began with relegation in 1985. In 1997 the federation expelled the club, and for two decades there was no top-flight basketball at Peñarol. The team returned to official competition in 2018 in the third tier, won the Metropolitano in 2019 and went up to the Liga Uruguaya de Básquetbol (LUB).

The climb back was not smooth. Peñarol reached the LUB finals in 2021–22 and again in 2023–24 and lost both. The Clásico against Nacional ran alongside it. Nacional won the first LUB-era meeting 85–67 on 18 March 2021, and by April 2026 the series stood at 8–8. The Palacio Peñarol, opened in 1955, has had a floating floor donated by FIBA since the 2011 renovation.
