Lokomotiv Gorna Oryahovitsa traces back to 1969, when engineer Todor Todorov set up a handball section inside the Lokomotiv sports society, taking the red, black and white of the wider Lokomotiv movement and the railway ties that gave supporters their name: Zheleznichari. Then, in 2022, the club beat Shumen 61 in Veliko Tarnovo to win its first Bulgarian Cup, and under head coach Yordan Dimitrov it kept going: a first league title in 2023–2024, defended in 2024–2025, and a third straight in 2025–2026, Shumen 61 again the team on the other side each time. A second cup followed in 2026, won 22:21 in Shumen's own arena.

Both clubs sit in northeastern Bulgaria, and their meetings are known there as the Derby of the Northeast. Stanislav Dobrev captained the side through the run of three titles and two cups before leaving for HK Lokomotiv Varna in September 2026 to become an assistant coach, closing out a captaincy that began in 2020. Ivan Petkov, a left back who captains Bulgaria's under-20 side, came back to Gorna Oryahovitsa the same summer after three seasons playing abroad.
