MLS Next Pro operates as the third tier of the soccer pyramid in the United States and Canada, sitting below Major League Soccer and the USL Championship. The league was established by MLS in 2021 to replace the former MLS Reserve League, creating a dedicated professional environment for player development. Unlike the USL Championship or USL League One, which consist of independent clubs operating as standalone business entities, MLS Next Pro teams function as extensions of their parent MLS clubs. This franchise model means there is no promotion or relegation, distinguishing it from the neighbouring second-tier competition focused on commercial independence.

The competition features a unique format for its country. As of 2024, 29 teams compete across two conferences, including 27 MLS-affiliated reserve sides and independent clubs such as Carolina Core FC and Chattanooga FC. The regular season comprises 28 games per team, played primarily within conferences with additional inter-conference fixtures. Playoffs involve a single-elimination tournament for the top eight teams from each conference, culminating in the MLS Next Pro Cup where the Eastern Conference winner hosts the Western Conference winner. Notable rules include the use of penalty shootouts to decide regular-season draws and specific suspension protocols for red cards.

New York Red Bulls II holds the record for the most titles, having won the cup in 2023 and 2025. They defeated Colorado Rapids 2 on penalties in 2025 after a 3-3 draw. Columbus Crew 2 won in 2022, while North Texas SC claimed the 2024 trophy. The league has never had a naming rights sponsor and remains officially known as MLS Next Pro. A transition to a fall-to-spring schedule is scheduled to begin in 2027. The league is operated by Pro Soccer Development, L.P., a subsidiary of MLS. In 2026, participating clubs include Atlanta United II, Austin II, Bethlehem Steel, Chicago FC United, Carolina Core, and Chattanooga.
